• Completed and signed Self-Certification Form - Individual / Joint Account

According to the guidelines of the Securities and Futures Commission, if customer prefer to send the account opening form by post, please use one of the following methods to verify client identity:

1. Arrange authentication by a certifier* to countersign the Account Opening Form, please also state his/her name, title and company name.
* The certifier must be a registered person, a Justice of Peace, or professional person such as bank’s branch manager, certified public accountant, lawyer or notary public.

2. Send the completed and signed Account Opening Form and other necessary documents, together with a signed cheque bearing your signature(s) and with your name(s) pre-printed on it and the cheque must:

- be drawn on your bank account with a licensed bank in HK;
- show your account name which must be pre-printed on the cheque. This account name must be the same as your name on the Account Opening Forms and your identity document/passport;
- be made payable to "Zhong Jia Securities Limited" with an amount not less than HK$10,000
- bear the same signature with the Account Opening Forms; and
- be encashed and cleared.

* The Company reserves the right to reject the application and require customers to provide additional documents for account opening.
